  • To confirm your identity, please send or hand carry one of the following documents together with the completed designated form.
  • • A copy of an official certificate such as a driver’s license or passport that confirms the identity of the person making the request. The certificate must display the name and address of the person making the request.
    • A copy of the requester’s certificate of residence (if residing in Japan) that was issued within 30 days prior to the date of the request.
  • If you are a proxy, please send or hand carry either (a) or (b) below in addition to the aforementioned completed designated form and the identification documents.

    • A copy of the proxy’s driver’s license, passport, or another official certificate that can confirm the proxy’s identity. The certificate must display the name and address of the proxy making the request.

The fee, which includes the cost of the reply from the Company, is as indicated below. Please enclose a postal fixed-amount money order to cover the fee when you mail the request documents (in Japan only).

• We will notify you promptly of the results of our internal review by one of the following methods.
